Skilled Nursing, Rehabilitation Therapies and Alzheimer Care

The Guardian Center is a 123-bed, not-for-profit, JCAHO accredited skilled nursing facility located on the Avon-Brockton line in Massachusetts. As you tour our site, you will find many extraordinary programs, services and people that provide a supportive therapeutic environment where the focus is on meeting our resident's individual needs.

Through our caring staff and comprehensive programming, The Guardian Center offers long-term care, rehabilitative care (speech therapy, physical therapy, occupational therapy) for stroke victims, those sustaining fractures, and people who need strength training following an injury or illness. Residents requiring diabetic care, and various IV treatments are also accepted. We also offer a secure Alzheimer's dementia unit and provide hospice care for the terminally ill.
